In some cases, insurance coverage has been denied or restrictions are placed on treatments to be included in the plan due to a pre-existing condition. While this practice is still common today among some coverage providers, there are situations where an individual's pre-existing condition can be included.
While there is no guarantee that any pre-existing condition can be included in coverage, most of the time options are available so that pre-existing conditions can be covered by either an increase in the premium cost (to account for the potential for future care of the condition) or by selecting a plan which can ignore all or almost all pre-existing conditions entirely.
Of course every plan and provider differs and many treat the pre-existing condition in their own manner, so it is best to discuss options for coverage with a qualified adviser before making a purchase.
Malaysia Pre-Existing Health Condition Insurance Coverage Options
While the easiest and most cost effective way to deal with a pre-existing health condition is to exclude it from coverage entirely and extend coverage for the rest of the individual or family's health exclusive of the condition, insurers understand that this is not always acceptable or practical.
Therefore, three primary methods of arranging coverage are available for anyone with a pre-existing health condition, as listed below.
Moratorium For Coverage – simply stated, a moratorium is a waiting period wherein the insured is covered against injury and illness for a pre-determined period of time (commonly around 24 months) while the pre-existing condition is excluded. At the end of that waiting period, if no significant symptoms or medical problems related to the pre-existing condition have arisen that require treatment, the policy will be adjusted so as to include the previously excluded condition in its coverage.
Premium Loading – though not as common as a moratorium, there are instances where an insurer will agree to increase the premium payment amount for coverage in exchange for extending coverage for a pre-existing health condition along with the rest of the policy benefits. This is not a very common practice as estimating the cost for potential treatment (should it be required) is very difficult.
Medical History Disregarded – this option is almost exclusively available through group coverage plans, often including more than 20 potentially insured members, where the need to offer the same coverage options for a wide range of individuals is necessary. As indicated by the name, it overlooks all past medical history and extends coverage as if the insured is in excellent health.
Please be aware that not every insurer offers these options for pre-existing condition coverage, and cost may vary greatly depending on insurer, specific condition involved, and other factors such as age. Most of the time, pre-existing conditions will need to be reviewed on a case by case basis to determine if inclusion is possible. For this reason, it is usually necessary to speak to an adviser prior to any purchase to find out what your options are if you need this type of coverage.

It is important to understand up front what amount of coverage you are entitled to prior to purchase of any policy, as waiting until an injury or situation arises (which may come near to or exceed the policy limit) is extremely stressful and potentially financially devastating.

As a general rule, all pre-existing health conditions present in and known of by the insured are usually counted as excluded at the start of a new policy.

Every insurer in Malaysia treats their policy deductible options differently and it is even possible to purchase a plan with no deductible at all, but the majority will have some form of deductible associated with their coverage as outlined in the policy conditions.
Simply put, a deductible is the amount of financial responsibility any person, family or group has toward their own healthcare treatments within a specific period or for a specific condition.

Type Of Deductibles For Malaysia Health Insurance
Co-Pay or Co-Insurance is the policy agreement wherein the insured agrees to pay a percentage of the needed treatment, medication and all related costs for covered healthcare they receive. To illustrate this; assuming the policy outlines a 20% co-payment for all medical treatments, this means that a for a treatment that costs RM 1000, the insured would be required to contribute RM 200 of their own money toward the treatment bill, while the insurer would pay the remaining 80%.
An Annual Deductible does not use a percentage to calculate the amount of financial responsibility toward their own health the insured bears, but instead sets a minimum amount that must be paid for all covered treatments before the insurer will begin to cover all subsequent expenses for treatment. This means that if the policy has a RM 2000 annual deductible then the insured will pay that amount for their healthcare in a policy year, and if any other cost is incurred above that amount it will be entirely paid for my the insurer.
A Per-condition Deductible is the most specific type of deductible commonly found in Malaysia because it outlines a specific amount of financial responsibility to be bore by the insured for each individual health situation. As an example; if an insured breaks their right arm and their pre-condition deductible is RM 500, then they will pay that amount toward their treatment and all other associated costs for the broken right arm will be paid by the policy, but if they break their left arm six months later in an unrelated accident then they will again pay RM 500 for the next condition.
Malaysia Health Insurance Claims
Anytime treatment is received by an insured, they will need to submit a claim to their insurer to notify them of the need for assistance. Without a claim submittal, there may not be any way for the insurer to know that they are responsible for any payment on behalf of their insured and the healthcare facility will often expect the treated individual to make payment personally for their care.
Depending on the nature of the treatment required, claims may be made by the insured (or in some cases on their behalf) by the treatment center. In general, whenever possible, it is best to contact your insurer prior to receiving treatment to ensure that all payments will be made.
Types Of Malaysia Health Insurance Claims
Several types of claims are used by the many insurance providers in Malaysia and most companies will even use different kinds of claims depending on the type of plan selected. At times, even the same type of coverage will have multiple claim forms depending on the treatment required.
In-patient Treatment Guarantee General Claim – this claim is often filed in advance of a covered non-critical treatment that the insured is planning to undergo in the near future. These claims alert the insurer to their need to make payment for an upcoming treatment and in most cases, once the claim is filed, all subsequent communication can be handled by the insurance company with the treatment center, proving very convenient for the insured since all they will need to worry about is the planned treatment.
It is important to file this claim in advance, because it is not uncommon for an insured person to be denied treatment for a non-critical condition by the healthcare facility if they do not receive prior approval from the insurer with promise of payment.
In-patient Emergency Care Claim – unlike the In-patient Treatment Guarantee General Claim, this claim is not filed in advance as emergencies are never planned. Instead, the insured should focus on preserving and restoring their good health, and afterwards can contact their insurer to deal with payment or reimbursement for treatment received.
Because not all healthcare facilities will be in every insurers healthcare network, it is vital that an insured familiarizes themselves with their cooperating healthcare facilities that are available in their area so that in case of an emergency they can seek care there if possible.
Out-patient General Claim – this form is usually filed to request reimbursement for out of pocket payment that was made for necessary out-patient treatment. This may include doctor visits or emergency treatment that does not require hospitalization.
Common Problems And Issues With Malaysia Health Insurance Claims
While insurance claims play an important role in making sure that all parties are promptly paid for needed treatments and coverage is always administered properly, the biggest complaint many have about their health insurance coverage is related to claims problems. As an insurance claim is a legal document, it is critical that claims are filled out, filed and handled properly. Read below to learn about some of most common issues concerning insurance claims.
Incomplete Paperwork Filing – probably the most common problem that slows the payment of treatment is due to paperwork not being completely filled out or important documents missing. Pre-existing Condition Exclusion – in the case where a pre-existing condition is excluded from coverage but the insured seeks medical treatment for the condition, then the claim for reimbursement will be denied. Malaysia Health Insurance Premiums
Malaysia health insurance plan's premium fees are typically calculated by the policyholder's age and the geographic coverage area of the plan. In some cases, some policy premiums are adjusted (usually increased) due to claims made for coverage and reimbursement, this is often called "experience adjustment”.

In general, premiums are estimated using a "community average”, which means that all insured in a particular location and age bracket will be charged the same rate for the same coverage. Financial Mediation Bureau (FMB)
The Financial Mediation Bureau (FMB) is an independent body set up to help settle disputes between you and your financial service providers who are its members.
The FMB provides consumers with a platform for an objective and timely resolution of disputes, claims and complaints arising from services provided by the member financial institutions (presently comprising the commercial banks, Islamic banks, investment banks, insurance companies, takaful operators, development financial institutions and card issuers that are regulated by Bank Negara Malaysia).
The FMB provides you with a fast, convenient and efficient avenue to refer your disputes for resolution as an alternative to the courts. These disputes may be banking/financial related as well as insurance and takaful related.
